New partners can introduce themselves without sharing resident information. Verified partners can sign in to submit authorized resident details and documents.

What to expect from a referral

  • Prompt acknowledgment of the introduction during posted business hours.
  • Clear explanation of Senior Resources' role and limits.
  • Required service, fee and relationship disclosures before referrals.
  • Consent for referrals and written authorization for clinical-record review.
  • Review by a Nevada-licensed RN of relevant H&P, discharge and continuity-of-care records.
  • A focused discussion of preferences, budget and location.
  • Status updates only when authorized and appropriate.

What to know before you refer.

Is Senior Resources currently licensed as a Nevada referral agency?

No. Applications for referral licensing related to Residential Facilities for Groups and commercial Assisted Living are in progress. This page supports partner outreach while that process is underway; placement activity will begin only after applicable licenses are issued.

Who can introduce a potential referral?

Hospital discharge planners, case managers, social workers, physicians and clinics, hospice teams, senior living professionals, veteran networks, concierge practices and other community partners may begin a conversation.

What should I include in the first email?

Include your contact information, the client's initials, general timing, current setting, preferred area and the broad type of setting being considered. Keep clinical and protected health information out of email; call for a secure next-step conversation.

Does a referral guarantee placement or admission?

No. Placement outcomes are not guaranteed. Each provider sets its own availability, assessment, pricing and admission criteria, and families make their own decisions.

Can a family contact Senior Resources directly?

Yes. A family may call 1 (866) 690-3303 or email info@careresourcesgroup.com to begin a general conversation about future placement support and licensing status.

When will license numbers appear here?

Once issued, the applicable Nevada license numbers will replace the “Pending” placeholders and this page will be updated to reflect the authorized scope.
