Not with searching every available property. This is the process Senior Resources is preparing to offer once the applicable Nevada approvals are issued.

    A discovery conversation

    We discuss the older adult's preferred area, budget, routine, accessibility, current support and future concerns.

    Clear service and fee disclosure

    We explain our services, funding, possible referral fees and any relevant relationships before anything else happens.

    Permission to proceed

    We obtain referral consent and separate written authorization for protected health information.

  4. 4

    RN review of relevant records

    A Nevada-licensed RN reviews the most recent and relevant prior hospital History and Physical (H&P), discharge summary and continuity-of-care records, then documents care considerations for the search.

    Focused options to consider

    We identify settings aligned with documented needs and family criteria. Each community confirms its own availability, pricing and eligibility.

    Comparison and tours

    We help organize questions and tours, with qualified professionals included when appropriate.

    The family decides

    The family decides whether, where and when to move. RN review does not replace the admitting facility's assessment or decision.

What helps at a first conversation: budget, location, setting size, accessibility, routines, timing and the authorized decision-makers. Please do not send clinical or financial details through a public form, ordinary email or text. After authorization, staff will provide the approved HIPAA-secure record pathway.
